Open Telerehab connects patients recovering from injury with their doctors remotely, with Quacker, a friendly duck-shaped guide, walking them through daily tasks, check-ins, and video calls.

The Design

Open Telerehab turns physical recovery into something patients actually want to open every day, pairing real doctor access with Quacker's daily nudges, streaks, and rewards.

Open Telerehab onboarding screen, with Quacker introducing himself in a chat bubble

Onboarding opens with Quacker's own greeting, not a wall of medical forms, so the first five seconds already feel like a friend showing up.

Open Telerehab home and activity screens, showing today's progress and full task list

Home and Activity sit side by side, today's progress on one screen, the full task list one tap away on the other.

Open Telerehab leaderboard and home dashboard screens

Recovery gets a leaderboard too, ranking patients by badges and Corns earned turns solo physio homework into something worth checking daily.

Open Telerehab doctor chat and video call screens

The actual telerehab part: patients message their doctor directly, then jump straight into a video call without leaving the thread.

The Process

Before Quacker and the app came together, the work moved from a stack of expression sheets and component cards to full assembled screens.

Daily task cards for a quiz, a satisfaction survey, and a hip-moving exercise

Daily tasks mix medical check-ins with actual exercises, quizzes and satisfaction surveys sit right next to reps and sets with Quacker.

Full mockup of the Open Telerehab home screen

The home screen looked nice, but somehow it didn't grab kid's attention to do their execises. It just an immediate attention.

Quacker's expression sheet, showing waving, worried, winking, and singing poses

Then we including more Quacker to the app, an expression sheet with pain scales, it worked great, but not what we expected.

Close-up of the home screen's task progress and Corns earned cards

So we came up with more component, Gamification. Corns, the in-app currency, and badges, the reward for completing tasks, are what patients actually chase, way more motivating than a plain progress bar.

A sticker from in-app message with doctor used will be bought with Corns they earned.

Close-up of the leaderboard screen with ranked patient avatars

Leaderboard avatars needed personality of their own, expressive enough to read as rivals, not just ranked names, boosting more engagement and competition between patients.

Row of achievement badges for breath, strength, focus, and grit

Achievement badges, breath, strength, focus, and grit, are what patients actually chase, way more motivating than a plain progress bar.
